COVID-19 Policies
The following policies have been put in place by Revival and the Oregon Health Authority to help reduce the spread of Corona Virus a.k.a COVID-19.
-You MUST stay home or reschedule if you have experienced symptoms of fever, shortness of breath or cough within the last 14 days of your scheduled appointment.
-You MUST stay home or reschedule if you have been in close contact with SOMEONE ELSE who has experienced symptoms of fever, shortness of breath or cough within the last 14 days of your scheduled appointment.
-Wear face mask throughout your appointment (unless instructed otherwise by technician).
-Arrive at your scheduled appointment time to reduce number of people in the building.
-If you're coming from working with the public, please change into clean clothes before your appointment.
